Stakeholder Engagement Sessions and Surveys

CDPHE’s Health Promotion and Chronic Disease Prevention Branch is currently engaging in strategic planning efforts to inform the direction of Colorado’s Tobacco and Chronic Disease programs. CDPHE maintains a strong commitment to health and racial equity as a foundation for all the work we do. Our vision is that every Coloradan has equitable access to opportunities to thrive regardless of who they are or where they live. Advancing health and racial equity through programmatic and policy efforts is our highest priority. Your input is valuable and needed as we collaboratively determine our path forward to advance our tobacco and chronic disease efforts.

We recognize that local public health agencies, health care providers, and community partners have many competing priorities at this time. CDPHE has worked with JSI Research and Training Institute, Inc. (JSI) to develop a wide variety of opportunities to learn from you and other partners across the state during the strategic planning process.

We encourage you to participate in any or all of the options described below, ranging from virtual engagement sessions to sharing your input through the tobacco and chronic disease planning surveys.

Social Determinants of Health

This 1.5 hour webinar presents a framework for developing and advancing community-led interventions that can change those social determinants of health which increase the vulnerabilities of communities and elevate the risks of people developing chronic disease. Chronic Disease Planning Survey (English)

CDPHE’s HPCDP Branch developed a survey to collect input and insights from our partners across the state. Questions focus on the social determinants of health, diabetes, cardiovascular disease, healthy eating and active living, health systems innovation and integration, and cancer. You will be asked to react to CDPHE’s current portfolio of evidence-based strategies and suggest specific strategies and approaches to reduce health inequities in rural areas, racial and ethnically diverse communities, and LGBTQ communities. Tobacco Planning Survey (English)

Questions focus on the social determinants of health, tobacco prevention policy, cessation, and health equity. You will be asked to react to CDPHE’s current portfolio of evidence-based strategies and suggest specific strategies and approaches to reduce health inequities in rural areas, racial and ethnically diverse communities, and LGBTQ communities. Facilitated Virtual Regional Sessions

These 2 hour virtual sessions are facilitated discussions and opportunities to share your input to inform the Colorado Chronic Disease and Tobacco State Plans. Sessions will either focus on chronic disease or tobacco (not both in one session).

Pre-registration is required for each session you participate in. Sessions are organized by region in order to account for regional differences across the state. We encourage you to participate in sessions scheduled for your region if possible. If not, please feel free to join other sessions that work better for your schedule. These interactive discussions will be conducted using the Zoom platform, along with Mural, a digital workspace for visual collaboration. It will be helpful to familiarize yourself with Mural prior to participating in these discussions.
