Chronic Disease Planning Survey (English)
Encuesta Sobre Enfermedades Crónicas (Español)
CDPHE’s HPCDP Branch developed a survey to collect input and insights from our partners across the state. Questions focus on the social determinants of health, diabetes, cardiovascular disease, healthy eating and active living, health systems innovation and integration, and cancer. Participants were asked to react to CDPHE’s current portfolio of evidence-based strategies and suggest specific strategies and approaches to reduce health inequities in rural areas, racial and ethnically diverse communities, and LGBTQ communities.
Tobacco Planning Survey (English)
Encuesta Sobre Tabaco (Español)
CDPHE’s HPCDP Branch developed a survey to collect information from stakeholders that will help to shape the tobacco-related goals and strategies for Colorado in the coming decade. Questions focus on the social determinants of health, tobacco prevention policy, cessation, and health equity. Participants were asked to react to CDPHE’s current portfolio of evidence-based strategies and suggest specific strategies and approaches to reduce health inequities in rural areas, racial and ethnically diverse communities, and LGBTQ communities.
Facilitated Virtual Regional Sessions
These 2 hour virtual sessions were facilitated discussions and opportunities to share your input to inform the Colorado Chronic Disease and Tobacco State Plans. Sessions focused on either chronic disease or tobacco (not both in one session).
